Community Health Liaison
KEY RESPONSIBILITIES
Strategic Leadership & Community Health Planning
- Build and maintain strong, respectful relationships with community members, including children, youth, families, and Elders.
- Conduct outreach to increase awareness of available health programs, services, and supports.
- Act as a trusted and approachable point of contact for community members seeking support or information.
- Promote participation in wellness initiatives, clinics, workshops, and community events.
Compliance & Governance
- Assist community members in navigating health, social, and community services.
- Provide referrals and connections to internal programs and external service providers, including clinical and allied health supports.
- Support individuals and families in overcoming barriers such as transportation, access, and system navigation.
- Advocate for equitable and timely access to care and follow up to ensure needs are being met.
Operations & Service Delivery
- Support the planning, coordination, and delivery of health promotion and prevention programs within the community.
- Assist in organizing and facilitating workshops, group sessions, and one-on-one education opportunities.
- Promote healthy lifestyles and wellness practices using holistic and culturally grounded approaches.
- Encourage early intervention and proactive health management among community members.
Key Program Areas
- Diabetes Education and Prevention: Support community-based education on diabetes prevention, management, and healthy lifestyle choices; assist with coordination of screening clinics, education sessions, and follow-up supports.
- Heart Health Education and Prevention: Promote awareness of cardiovascular health, including nutrition, physical activity, and lifestyle changes; support initiatives that encourage active living and heart-healthy practices.
- Communicable Disease Education and Prevention: Support public health education related to communicable diseases, including prevention, vaccination awareness, and harm reduction; assist with outreach during public health initiatives or response efforts.
- Nutrition and Dietitian Services: Support access to dietitian services, including referrals and coordination of appointments or community visits; promote healthy eating, food security initiatives, and culturally appropriate nutrition education.
- General Wellness Programming: Support programs that address mental wellness, substance use prevention, family wellness, and overall healthy living; assist with culturally grounded and land-based wellness initiatives where appropriate.
Internal Communications & Organizational Leadership
- Support individuals and families in addressing barriers related to housing, food security, income, transportation, and access to care, and connect members with appropriate supports.
- Identify service gaps and emerging community needs and communicate these to leadership.
- Work collaboratively with multidisciplinary teams (e.g., nurses, dietitians, mental health workers, and community service providers) and participate in team meetings, case discussions, and coordinated care planning.
- Promote culturally safe, trauma-informed, and inclusive approaches; support integration of traditional knowledge, cultural practices, and community strengths into programming, including engagement with Elders and knowledge keepers.
- Assist with planning, coordination, and evaluation of community wellness programs; maintain accurate records and documentation; and identify opportunities for continuous improvement.
- Participate in community events, committees, and organizational initiatives, and perform other related duties that support the overall health and wellbeing of the community.

EDUCATION & EXPERIENCE
- Diploma or certificate in Community Health, Social Services, Human Services, or a related field, or an equivalent combination of education, training, and experience.
- Experience working in community-based health, wellness, or social services programs.
- Experience working within Indigenous communities or organizations is strongly preferred.
- Knowledge of local health systems and community resources is an asset.
- Class 5 Drivers License
WORKING CONDITIONS
- Community-based role with regular outreach and engagement within the community, including attendance at clinics, workshops, and community events.
- May require flexible hours, including occasional evenings and/or weekends, based on program and community needs.
- Work involves interaction with individuals and families who may be experiencing complex health and social challenges; requires a trauma-informed, culturally safe approach.
